Dental Patient Information – Hamden, CT

Your Dental Needs
are Our Top Priority

At the dental office, our staff will strive to make you feel welcomed and right at home from the moment you step through our front door. Our top priority will be to understand the reasons for your visit and your personal dental goals as well as possible, helping you find the relief and fulfillment you need to smile with confidence through experienced care. At our dental office, we are proud to be fluent in English, Italian, and Spanish.

Your First Dental Visit

Once you arrive, you’ll be pleasantly welcomed by our front desk, then introduced to a dental assistant who will bring you to one of our operatories. A dentist will come in shortly, introduce themselves, then give a brief rundown of how the dental office uniquely operates. After asking questions about your oral health and giving you a chance to discuss any concerns you have, a series of X-rays will be taken and a cleaning will be performed, either by the dentist or a dedicated hygienist.

New Patient Forms

We have made our new patient forms available to you as Adobe Acrobat files. For your convenience, we recommend printing and completing these forms at home. Please remember to bring your completed forms with you for your first appointment. If you have any problem getting these files to print properly, please call our dental office and we will gladly send or fax them to you.

You may also complete the new patient forms online and submit them directly to us. Please click the button below to complete the forms online.

Dental Insurance Welcome

Our dental office participates with some Aetna, Cigna, Anthem Blue Cross, and Delta Dental insurance plans. No matter what dental insurance you have, however, we’ll be happy to file your claims for you.

We know that your insurance may be essential to your dental care. That’s why we recommend that you review your insurance policy before your appointment. Many services that are necessary for your dental health are not covered under some plans. Patients are responsible for paying deductibles, co-pays, and other balances not paid by the insurance company.

We hope that you can choose the care you need when you need it and not when your insurance company dictates it. If you need extensive or long-term care, we offer CareCredit to help you finance your treatment. We’re always happy to answer your finance or insurance questions during your appointment or when you call us today.

We proudly accept the following dental insurance plans:

  • Aetna
  • Anthem Blue Cross
  • Cigna
  • Delta Dental

Ask our friendly team if we take your plan.

Bright Smile Club

Wish your dental care was more predictable? When it comes to long-term oral health, getting preventive services like checkups and cleanings completed is one of the most important things you can do for your smile. Our Bright Smile Club allows patients who would prefer to avoid dental insurance to pay for care in monthly or annual installments and gain access to services at predictable rates. Since it’s not dental insurance, you don’t have to worry about claim forms, deductibles, annual maximums, waiting periods, or speaking to insurance agents over the phone!
